To get started, all clients are required to set up their profile in our secure system. Please complete your intake forms using the link below before your appointment:
Once your purchase and intake forms are complete, a member of the Perform Rx team will send you your unique reference number. This number is required to activate your test.
Visit the activation page below and enter your unique reference number when prompted.
After submitting your number, you will receive detailed instructions and a shipping label for your sample.
Cut five or more small strands of hair and place them into a small plastic bag. Hair can be collected from the scalp or body, including beard or leg hair. Pubic hair cannot be used. Hair products and hair dye do not affect the test.
Place the plastic bag containing your hair sample into your own envelope. Clearly write the provided lab address on the envelope, apply postage, and mail your sample according to the instructions provided during activation.
Once the lab completes your analysis, your results will be sent to Perform Rx. Our team will review your report and guide you through your results and recommended next steps.
